By checking the box below, I hereby provide explicit consent for Liberty Shield (“the Service Provider”) to act on my behalf to request the deletion, suppression, blocking, or monitoring of my personal information across data brokers, platforms, websites, and other third-party services, as applicable.

I authorize Liberty Shield to use my personal information (including, but not limited to, my name, email address, phone number, mailing address, and government-issued identification) solely for the purpose of verifying my identity and fulfilling these requests.

I understand that Liberty Shield will make reasonable efforts, using industry-standard practices, to reduce my digital exposure through data broker removals, biometric suppression, spam reduction measures, and dark web monitoring. However, I acknowledge and agree that results may vary depending on third-party compliance and factors outside Liberty Shield’s control. Accordingly, Liberty Shield cannot guarantee:

• Complete or permanent deletion of personal information from all sources;
• Universal suppression of biometric or facial recognition profiles;
• Elimination of all spam calls, texts, or emails; or
• Detection of every instance of compromised data on the dark web.

I further understand that I may withdraw my consent at any time by contacting Liberty Shield customer support.

By checking this box, I affirm that my electronic acceptance constitutes my legal signature and carries the same force and effect as a handwritten signature.
